HOW TO PRINT COMPLETED REGISTRATION FORM:

Please login to the registration site again.  Click on your children’s profile (for each one of them) under Sub-Accounts.  Once you click on the profile, click the  “Forms” button on the left hand side.  Now click on the “2008 Player Registration Form” link on the right hand side.  It should open up a “pdf” file with all of the information entered by you.  Open up the file using Adobe Acrobat Reader.  if you do not have Adobe Acrobat Reader, you may download it for FREE by clicking here.  Please print these forms and submit them to MGAYSO through one of the following methods:

·         Drop off:  You can drop off the form for the next two Saturdays (May 31 or June 7, weather permitting) to any adult at the AYSO Tent during game time (10 AM – 3 PM)
 
·         Bring it to our next in-person registration event (see dates and times in the home page).
